On the recommendation of a doctor from the hospital, you need to undergo a cardiologist. Since there is an incoming cardiologist in the 27th polyclinic, the therapist issues the coupon. My district therapist, AliMohamed, told me to get examined and wait for a call from him when there would be a ticket to the cardiologist.
