Medical certificate is not accepted from private clinics, only from the city. At the same time, the 9th polyclinic said that only the 4th and 9th polyclinics have a license to issue certificates for a residence permit. I could not find an up-to-date questionnaire on the Internet, the employee issued a form, I filled it out on the spot. The receipt of payment of the state duty must be stamped at the bank.
